CHECKLIST FOR FILING 2011 TAXES
1. Collect all W-2’s.
2. Interest and Dividend statements.
3. Mortgage interest statements from banks and mortgage companies.
4. Settlement sheets from sale or purchase of any real estate.
5. Form 1099-MISC for work you performed as a subcontractor.
6. Form 1099-C from Relief of Debt.
7. Form 1099-A for Acquisition or Abandonment of Secured Property.
8. Form 1099 for Proceeds from Unemployment Benefits.
9. Name and social security number of child care provider and amount paid for childcare.
10. K-1’s from partnerships, corporations or fiduciaries.
11. Income and expenses from investment property.
12. Statements from broker showing purchases and sale of stocks, bonds, etc.
13. If self-employed, all income and expenses related to self-employment.
14. If using a portion of the home as an office, calculate the percentage of the home being used and use that percentage of the total insurance, heat, utilities and other costs or improvements associated with the operation of your office.
15. All business expenses related to seeking new employment even if the search was unsuccessful.
16. IRA or pension distributions, and any IRA, SEP or SIMPLE Plan contributions you made.
17. Alimony payments you made or received.
18. Vital information for Domestic Help (Social Security #, Payroll, Withholdings).
19. Charitable Contributions. Please list- no receipts.
20. Expenses related to the Adoption of a Child.
21. College and Post-Graduate tuition payments and Student Loan interest.
22. Name, social security number and date of birth for all dependents.
23. Sales tax paid on a large purchase such as automobile or boat.
24. Moving expenses if related to job change.
25. Out of pocket medical expenses.
26. If you choose to directly deposit your refund, please include a VOIDED check.
27. Cobra benefits paid.
28. Energy efficient improvements made to primary residence.
29. Please include e-mail and contact numbers.

• 2011 Gross Income.
• 2011 Itemized Expenses.
• New assets purchased in 2010 (purchase price and purchase date).
• Year end, 2011 cash balances.
• Annual salary of officers and employees.
• 2011 payroll taxes paid.
• List of outside contractors, relevant federal ID#s and amounts paid to each.
• Prior year’s depreciation schedule.
• Auto mileage.
• 2011 year end loan balances.
• 2011 interest expense paid.
• 2011 shareholder draws (for Subchapter S corporations).
• Start-up Expenses.
• 2010 Financial Statements.
• 2010 Income Tax Return.
• Employer/Employee contributions to Retirement Funds.
